Case Managers act as an advocate for their client’s health and make sure they receive the right care for their situation. The Case Manager works with healthcare professionals like Doctors and Nurses to come up with a treatment plan that best fits their client’s needs. The Case Manager also is responsible for duties assigned by the Psychiatric Services Manager.
Case Managers arrange transitional plans for clients to continue receiving excellent care after being discharged from the hospital/mental health center. Transitional plans ensure that clients continue to improve even without the constant care provided in a community mental health setting.
Essential Responsibilities and Metrics:
- Provide face-to-face mental health annual assessments when necessary
- Facilitate client referrals for additional services consistent with assessed needs physician recommendation and/or client desired outcomes
- Complete referral, transfer, and discharge paperwork in timely manner: referral and transfer, immediately upon need; discharge summary and instructions within three working days of termination
- Provide brief crisis intervention, stabilization services and supportive counseling to meds only clients
- Regularly consult with psychiatric providers and/or Psychiatric Services Manager on specific client related clinical issues
- Under direction of psychiatric providers, case manager will provide clients medication education print outs that are focused on understanding illness, symptom management and medication compliance
- Under direction of psychiatric providers, case manager will facilitate psychiatrist signatures on appropriate paperwork
- Under direction of psychiatric providers, case manager will provide clients with lab request form, medication consent form and controlled substance agreement and obtain client signatures
- Educate clients on how to initiate and participate in a telehealth visit
- Call clients in advance of scheduled telehealth visits to ensure they are trained and confident in their ability to participate, and to confirm availability for the visit
- Schedule clients follow up appointments
- Take client vitals during in-person appointments
- Refer client to Psychiatric Services Manager when appropriate
- Provide back-up coverage for Psychiatric Services Scheduler and Medication Line
- Assumes leadership role either by own initiative or when asked by Psychiatric Services Manager
- Complete Service Ticket/Progress Note within three working days of day of service
- Provides high-quality service and maintains 50% of direct service hours per month
- Receive input from the Psychiatric Services Manager and provide feedback regarding administrative issues
- Participate in supervision with Psychiatric Services Manager
- Participate in Staff Development seminars
- Attend weekly case management meetings
- Clear vacation and time off request with Psychiatric Services Manager in a timely manner
- Attend weekly case presentation meetings when possible
- Appropriate and timely response to Psychiatric Services Manager requests for data or special projects
- Participate in orientations and training programs of the Center
- Protect client confidentiality and abide by the ethical statement of the agency
- Other duties as assigned by Psychiatric Services Manager
Qualifications:
- Bachelor’s degree in related field
- Strong verbal and written communication skills
- Computer literacy to maintain and manage case records
- High organizational skills to manage several different cases at once
