Job Summary

Psychiatric Services Coordinator is responsible for supporting the RN, Psychiatric Services Manager in the oversight of the Psychiatric Services Team.

Essential Responsibilities and Metrics

  • Provides high-quality service and maintains 35% of direct service hours per month
  • Participate in the onboarding process of new Psychiatric Services Case Managers
  • Provide training to new Psychiatric Services Case Managers
  • Provide support to all Psychiatric Services Case Managers
  • Monitor Medical Services emails on Outlook and ensure all emails are addressed promptly on a daily basis
  • Monitor DCFS tracking spreadsheet and ensure DCFS medication consent forms are up to date
  • Provide thorough support to clients in need of intensive care coordination
  • Facilitate client referrals for additional services consistent with assessed needs, physician recommendation, and/or client desired outcomes
  • Regularly consult with psychiatric providers and/or Psychiatric Services Manager on specific client related clinical issues
  • Provide case management support to Medicare, med only clients
  • Provide brief crisis intervention, stabilization services and supportive counseling to Medicare, meds only clients
  • Participate in supervision with RN, Psychiatric Services Manager
  • Attend weekly psychiatric services case management meetings
  • Clear vacation and time off request with RN, Psychiatric Services Manager
  • Complete all clinical documentation and required reports in an accurate and timely manner
  • Complete Service Ticket/Progress Note within three working days of day of service and at the end of each week
  • Participate in orientations and training programs of the Center
  • Protect client confidentiality and abide by the ethical statement of the agency
  • Other duties as assigned by RN, Psychiatric Services Manager
  • Assumes leadership role either by own initiative or when asked by RN, Psychiatric Services Manager

Qualifications

  • Bachelor’s degree in related field
  • Strong verbal and written communication skills
  • Computer literacy to maintain and manage case records
  • High organizational skills to manage several different cases at once
